8.6.4.1 To Change a Form Status

Once a form has been associated on a Study or Patient level, the form status must be “Active” before it can be used.
 
To change a form status:
1.    Click the Status link for the desired form.
The Form Status page displays:
 
 
2.    Use the dropdown menu to set the appropriate status.
3.    Enter your e-Signature and click Submit.
